On May 10, 2019, Inmate Castro returned to Dade Correctional Institution from Larkin Hospital.
The standard protocol for any Inmate returning from an outside hospital is to have a complete evaluation by the Infirmary Nurse. If the Inmate is received after hours, the on-call provider needs to be notified.
At 0230 Inmate Castro was received without a Nursing report to Inpatient Mental Health Unit for Self Harm Observation Overflow. During Nursing rounds, it was discovered Inmate Castro had multiple ACE wrap bandages on his wrist, ankles & knee. Nurse Wilnie Chery,RN Medical Infirmary Nurse not only falsified the Nursing documentation but was negligent by letting this Inmate have a tool to attempt to commit suicide while on Suicide Watch.
Inmate Miles, Rodney DC #R47766 was serving time at Dade Correctional Institution for murder.
On May 10, 2019, Inmate Miles, Rodney was on Self Harm Observation Satuts housed in Medical Infirmary.
At approximately 0520 Inmate Miles was received without a Nursing report from Medical Infirmary to Inpatient Mental Health Unit as a Suicide Watch Overflow.
Inmate Miles, Suicide Watch Observation Check List (DC4-650) was completed by Medical Infirmary Nurse Wilnie Chery,RN until 0800; falsifying this Inmates Nursing documentation.
Observation Self Harm Observation Checklist (DC4-650) Completed by Nurse Wilnie Chery,RN. As you can see Nurse Chery falsified 2 hours & 40 minutes of 15 minutes checks by documenting his meals, fluids & verifying Inmates smock/gown/blanket/mattress checked (REQUIRED ALL SHIFTS) at 0800.
Inmate Delany, Daniel DC# V53084 was serving time at Dade Correctional Institution for sex crimes against a minor child under the age of 12.
A St. Johns County inmate is now facing three charges of capital sexual battery, the St. Johns County Sheriff's Office said Thursday.
On July 20, 2018, Inmate Delany was housed in The Inpatient Crisis Stabilization Unit on Self Harm Observation Status for suicidal ideations.
Although it was reported that Inmate Delany, Daniel had on boxers while on 15-minute checks for Self Harm Observation Status, Inmate Delany successfully committed suicide by strangling himself with the same boxers just 1 day later on July 21, 2018.
What standards & ethics do Nurses have while working at this Level 5 State Correctional facility? NONE.
Although all of these Incident Reports were turned directly into the Director of Nursing, Assistant Director of Nursing & Security Supervisors; all of this was ignored. Why?
Now, Centurion of Florida, LLC could not afford to admit they hired incompetent Nursing staff that had a history of falsifying Suicide Watch Documentation & did absolutely nothing about it. That would have jepordizied the $644,656,815.46 Contract & $1,639,837,412.00 Contract with the Florida Department of Corrections.
The only care, custody & control the Florida Department of Corrections has is covering up its own corruption.
